“FATBURGER SUNDAY FOOTBALL HOME INVASION”

 

 

 

1.             How to Enter the Contest:

 

a.         To participate in the contest, you must register at fmtalki.com between Friday, August 29^th, 2003 at 12:01am and Friday, December 26^th, 2003 11:59pm.     To enter on-line, you must enter your first name and last name, complete address (city, state, zip code), home telephone number and a valid e-mail address in the on-line entry form.  One Internet entry per person and one Internet entry per e-mail address.  Internet entries will be deemed made by the authorized account holder of the e-mail address submitted at the time of entry.  The authorized account holder is the natural person who is assigned to the e-mail address by an Internet access provider, on-line service provider or other organization that is responsible for assigning e-mail addresses or domain associated with the submitted e-mail address.  All entries must be received by 11:59am on Friday, December 26^th, 2003.  KLSX will pick a new winner every week.

a.                   KLSX is not responsible for entries not received due to difficulty accessing the Internet, service outages, or delays, computer difficulties, or other technological glitches.

2.             Eligibility Restrictions:

 

a.                   The contest is open to all KLSX listeners who are 21 years of age or older residing in the Los Angeles metropolitan area.  Employees of KLSX, its licensee Infinity Broadcasting Corporation, corporate parent company Viacom Inc., affiliated entities, their affiliated advertising agencies, participating sponsors/ promotional partners, other radio stations in the Los Angeles metropolitan area and the members of their immediate families, ABC Inc, Fat Burger Inc, their distributors, affiliates, subsidiaries, advertising agencies, promotional agencies, their immediate family members and/or those living in the same household of each and employees of retailers are NOT ELIGIBLE  to participate or win.

b.                   Listeners are eligible to win a KLSX contest only once every sixty days.  Only one winner per household is permitted in any contest.  Listeners are eligible to win a KLSX contest prize valued $600.00 or more only once every six months.

c.                   Contestants are required to provide truthful information and KLSX will reject and delete any entry that it discovers to be false or fraudulent.  KLSX will also disqualify and delete any entry received from the persons under the age of 18. 

3.                    Internet Contest Privacy Policy:

a.                   By use of this website and by entering the contest, you agree to KLSX’s Terms of Use Agreement and to the use of your personal information as described in the Privacy Policy, which are located at: fmtalki.com.

 

4.             Prizes:

 

a.                   Each week the winner will receive a 27” inch Flat screen Television with an approximate retail value of $800.00, a one hundred dollar ($100.00) gift certificate to Fat Burger restaurant.  All winners of the certificates will be subject to the terms and conditions as set forth by the provider of the gift certificate. 

b.                   Contest prizes are not transferable.  Prizes may not be substituted for or redeemed for cash. 

c.                   Winner agrees to sign a liability waiver releasing Infinity Broadcasting from any liability.

d.                   All prizes or prize certificates may be picked up at the office of KLSX, 3580 Wilshire Blvd. Los Angeles, CA 90010.  Any prize or prize certificates not claimed within forty-five days of winning will be forfeited by the winner.  In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, the winner assumes the risk of its arrival.  KLSX is not responsible for the safe arrival of a prize or prize certificate.

5.             Selection of Winners:

 

                a.                Decisions of station management with respect to the contest are final.

b.             One (1) winner will be selected every Wednesday at 12 noon for the duration of the contest.  KLSX will randomly select one (1) entry from the eligible entries received.

c.             Listener must register online.  The odds of winning depend on number of listeners registering on KLSX.   

e.                   Winner will be notified by KLSX within 1 day and given further information.  If the winner cannot be contacted within 1 day after the prize is awarded, an alternate winner will be selected in a random drawing of all the remaining eligible entries and will be notified by telephone.

Conditions:

 

a.             Payments of all federal, state and local taxes are solely the responsibility of the winner.  Winners may be required to sign an IRS Form W-9 or the equivalent.

b.             By participating in the contest, the winner and his/her guest (who must be 21 years of age or older) agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest, without further financial or other compensation, and to sign a publicity release confirming such consent prior to acceptance of the prize.

c.             Contest winner and his/her guest will be required to sign a liability release prior to acceptance of any prize valued at $600.00 or greater or which prize includes the provision of transportation, whether or not such transportation is provided by KLSX.   Pursuant to the liability release, the winner and his/her guest will agree to hold KLSX, its corporate licensee, Infinity Broadcasting Corporation, parent corporation Viacom Inc., affiliated companies and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest.

 

d.             KLSX reserves the right to make changes in the rules of the contest, including the substitution of a prize of equivalent value, which will become effective upon announcement.  KLSX is not responsible for any schedule changes, postponement, delays or cancellations due to any problems beyond its control.

 

e.                No purchase is necessary.  The contest is void where prohibited.

 

f.              KLSX in its sole discretion, reserves the right to disqualify any person tampering with the entry process, the operation of the KSLX web site or who is otherwise in violation of the rules.  The station further reserves the right to cancel, terminate or modify the contest if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.

f.                    Failure to comply with the contest rules may result in a contestant’s disqualification at the sole discretion of KLSX. 

g.                   KLSX is not responsible for typographical or other errors in the printing offing or the administration of the contest or in the announcement of prizes. 

 

h.                   Copies of the written contest rules and a list of winners (when complete) are available during regular business hours at the main studio of

KLSX, 3580 Wilshire Blvd.  Los Angeles, CA  90010 or by sending a self-addressed stamped envelope to KLSX
